Sustaining current improvement in the post-project period will be linked to the degree to which changes have become embedded in the care system, i.e. whether they are transformed from ‘interventions’ to routine practice.

Sustaining current improvement will depend on factors such as:

  • Enthusiastic clinician engagement;
  • Increased knowledge and skills of team members;
  • Redesign of existing work practices;
  • Management support;
  • Demonstrated positive results;
  • Resource implications; and
  • Clear change in culture and attitudes.
Planning for sustainability needs to begin early in the project, rather than as a fixed end date rapidly approaches.
 

 
  There is a vital need to engage management and consumers early on, to keep these two groups up to date, to allow them some ownership in guiding the project and in problem solving during the project.

The advantages of this approach would be different perspectives during the project's life that may assist with implementation and importantly assistance in solving the sustainability issues when a project (which had positive outcomes) is close to an end.
