How can this model be implemented?

Implementation involves completing a number of key tasksfrom the CSS model, although the exact order in which you complete them may vary between projects. These tasks can be grouped into the following broad phases:

 



   
  1. Project design

The project design phase involves defining the main features of your project and forming your multidisciplinary team. Tasks involved in this phase include answering the following key questions:
  • What are potential topics of interest?
  • What are you trying to achieve?
  • How are you going to go about it? and
  • Who needs to be involved?

 
 
 

   
  2. Measurement and review

The measurement and review phase can serve the following two functions:
  1. Investigating current practice; and
  2. Providing a diagnostic picture of any problems and the extent of these problems.
The key questions involved in establishing a baseline picture of current practice are:
  • What processes of care make up current practice? and
  • How congruent are they with the best available evidence?
Answering these questions requires defining the individual steps in care, and making decisions about:
  • the evidence;
  • the indicators against which current practice will be measured; and
  • the methods for collecting and analysing the data.

 
 
 

   
  3. Intervention

The intervention phase involves designing practice improvement interventions and implementing them.

Tasks in this phase include deciding how you can narrow the gaps you’ve identified between current practice and the best available evidence.

 
 
 

   
  4. Evaluation

The evaluation phase involves comparing new practice with the baseline picture.

The task here is to evaluate the impact of your interventions and see if you have made a difference.

Depending on your project time frame and results, your project may repeat phases 3 and 4 several times.

 
 
 

   
  5. Sustaining better practice

This phase involves examining what you can do to sustain better practice beyond the project’s time frame and into the future.
