The Clinical Support System model
The Clinical Support System model aims to maximise patient
outcomes by embedding the best available evidence routinely
into clinical practice. The CSS model provides a broad
framework upon which clinical teams, in partnership with consumers, can design a project
to understand and systematically analyse the care they provide for a defined cohort of patients.
Based on the results they gain, teams can then develop and implement
changes to produce sustained improvements in their practices.
The CSS model consists of the following simple elements:
- A focus on a defined cohort of patients;
- A multidisciplinary team-based approach to patient care;
- Systematic use of the best available evidence to inform,
measure and review the care provided;
- Ongoing action to address any identified gaps between
evidence and practice; and
- Consumer involvement.
These elements can be applied to a range of clinical conditions
and care settings, and can accommodate a variety of tools
and strategies to improve practice.
