Applying the CSS model to your project

Each project will be unique, based on a combination of such factors as:

• Your chosen patient cohort and the existing evidence base to support clinical practice for this group;
  
• The breadth of the particular care system in which your project is implemented;
  
• The specific features of that care system, e.g. the organisational structure and care processes, and the staff profile;
  
• The objectives of your project and the outcomes sought;
  
• The composition of your multidisciplinary team;
  
• The availability of existing consumer groups;
  
• The resources available to your project such as funding and the skill mix of staff involved; and
  
• The consequences of all the decisions made during the design and implementation stages of your project.
